The challenge
A support worker finishes the visit, then spends 20–40 minutes typing it up. By the third one of the day the detail thins, the wellbeing fields get skipped, and the audit-relevant bits disappear into free-text.
A regulator review pulls a sample, finds two notes missing safeguarding-relevant detail, asks why. The worker can't remember; the documentation says nothing. The provider gets a finding for something that wouldn't have happened with a prompt at the time.
What the system says about a resident is six weeks behind what the team actually knows. Important changes, medication adjustments, family contact, behaviours, sit in a worker's head and a sticky note, not the record.
A new support worker takes a shift. Twenty pages of case notes to read; the actually-important context isn't flagged. They run on instinct for the first week, and on a vulnerable cohort that's a risk.
What we build
Support worker speaks the note at the end of a session, phone, tablet, or laptop. AI structures the speech into the right note template (key-worker session, daily log, incident report, medication round), in your provider's tone, ready for sign-off.
As the worker writes (or speaks) the note, AI checks against the resident's active care plan + your compliance schema. Missing fields prompt back: wellbeing observation, medication confirmation, safeguarding flags, behaviour markers. Caught at the time, not at the audit.
Every note gets a transparent compliance score against the provider's standards (and where applicable, CQC-aligned domains). Low-scoring notes flagged for the manager before close. Trends per worker, per home, per shift type visible at a glance.
Approved notes auto-update the resident's case file: care plan annotations, risk-assessment flags, key-worker reviews. The record of truth stays current without anyone manually copy-pasting between systems.
Per-resident document tracking: care plans, risk assessments, mental capacity assessments, DoLS authorisations, GP letters, family agreements. Renewal dates monitored, overdue items escalated, evidence stored against the record.
Auto-generated per-resident summary for the incoming shift: what changed since last handover, open actions, safeguarding flags, behaviours of note. Worker reads two paragraphs, not twenty pages.
Family update emails, GP letters, social worker reports drafted from the case file. On-brand, factual, with the worker reviewing before send. Multi-channel where appropriate.
Per-worker tracking of mandatory training (safeguarding, medication, first aid, fire), DBS renewals, supervision dates, appraisals. Escalating reminders before anything expires.
